Application for Permanent Residency are required to bring with them:

Form 26 (medical)
Form 160 (X-ray)

2 passport photos + passport
(1 for X-ray form)

(Any medical report on operations or current medical conditions)

Temporary residents may only be required to have a chest X-ray, not a medical.

Charges (including VAT and postage):

• Under 11 - £64.00
• Medical & CXR for applicants 11 & over - £204.00
• Medical CXR & HIV for applicants 15 & over - £254.00
• Doctors, dentists, nurses also require HepB (surface antigen) & Hep C - £90.00
